A gold color metal and enamel device 1 1/8 inches (2.86 cm) in diameter overall consisting of a gold lion on a maroon cross with top bar crenellated and lower bar enclosing a red cross of St. George of England issuing from beneath the lion's dexter rear paw, the whole encircled by a gold scroll with a green palmetto tree and white crescent moon issuing above the crenellations, and on the scroll the motto "QUALITY DEVOTION PRIDE" in red letters.
The medical mission of the unit is represented by the maroon cross. Charleston's historic role in the defeat of the British in one of the first battles of the American Revolution is recalled by the crenellated fortress, the palmetto tree used to build the fort, and the red cross of England shown below. England's historic association with Charleston is represented by the lion of England and the state of South Carolina by the crescent moon and palmetto tree from the State flag. The mission of the unit is symbolized by the colors maroon and white: red represents historic ties with England and green stands for care and healing.
The distinctive unit insignia was approved on 7 January 1986.
